Low CTR - Ways to improve?

I have local client, small insurance agency who does health insurance, life insurance, and small business plans.

They are ranking incredibly well locally. Almost every main term (life insurance city, health insurance city) they are in the top spot for locally and many they are in the map pack and normal organic results.

But, our click through rates are really low.

Example: from GSC, for the term “medical insurance” they have 400 impressions with 1 click (avg position 4.21). They show up on map pack, and have 2 pages (home and product page) on the first page (usually around 4-6 spot).

Outside of adjusting Title and Meta, is there anything else I should be looking at to help improve CTR?
